Responsibilities: - Collaborate with a multidisciplinary healthcare team, including physicians, occupational therapists, physical therapists, social workers, speech therapists, family members, caregivers, and home health aides, to establish, implement, and revise a personalized plan of care. - Ensure that the delivered services align with the plan of care by continuously assessing, planning, coordinating, counseling, teaching, and supervising the care to meet the patient's needs. - Provide oversight and supervision to Licensed Practical Nurses and Paraprofessionals, conducting regular home visits to comply with regulatory and VNS Health requirements. - Monitor and assess the patient's condition, communicate with the physician and the healthcare team about any changes in the patient's needs, and update the treatment plan accordingly. - Manage, plan, coordinate, and execute the patient's safe discharge from our services, ensuring adherence to VNS Health and regulatory requirements in terms of physician's orders, discharge summaries, and communication with the team and family members. - Communicate effectively with the patient's primary care physician, addressing any issues that may impact the patient's care, such as abnormal clinical findings or medication discrepancies. Take an active role in coordinating all aspects of the assigned caseload. - Assist patients with their activities of daily living, which may involve positioning, moving, transferring, and lifting patients of varying weights and physical conditions. Provide support and assistance as needed, taking into account the preferences of the patient, family members, and paraprofessionals. Qualifications: - Education: Completion of a nursing degree from an approved program is required. - Work Experience: Minimum of one year of experience in a medical/surgical environment is required, preferably including experience with Community Health Assessment and Assistance (CHAA). - Licenses and Certifications: Current registration to practice as a registered professional nurse in New York State, along with a valid Nurse License, is required.
