What You’ll Do:
- Assess: Evaluate the psychosocial status of patients and their families, communicating findings to the interdisciplinary team to ensure holistic care.
- Counsel: Provide support for patients and families dealing with stress and coping difficulties; offer crisis intervention as needed.
- Educate: Guide patients and families in preparing advance directives and understanding their rights and resources.
- Collaborate: Work closely with community agencies, maintaining relationships to enhance patient support and care.
- Participate: Contribute to the development of individualized care plans and engage in interdisciplinary meetings to address psychosocial aspects of terminal illness.
Qualifications:
- Education:
* Master’s degree in Social Work (MSW) accredited by the Council on Social Work Education, or
* Bachelor’s degree in Social Work with relevant qualifications as stated in Medicare guidelines.
- Experience: Minimum of one year in healthcare; home health/hospice experience preferred.
- Skills: Strong verbal and written communication abilities, good organizational skills, and a deep understanding of hospice philosophy and issues surrounding death and dying.
- License: Current licensure to practice in the state of operation; valid driver’s license with a reliable vehicle that meets insurance requirements.
