Key Responsibilities:

Patient Evaluation and Treatment:

• Assess patients’ physical capabilities and limitations, developing personalized care plans that enhance mobility and reduce pain.
• Implement individualized treatment plans with therapeutic exercises, manual therapy, and various modalities to improve functional abilities.

Patient Education and Support:

• Educate patients and their families on the benefits of physical therapy, providing support throughout the recovery process.

Collaboration:

• Partner with healthcare professionals, such as physicians, nurses, and occupational therapists, to foster a collaborative environment that supports patient recovery.

Documentation:

• Maintain accurate patient records and ensure effective communication with other healthcare providers, supporting continuity of care.

Professional Development:

Qualifications:

• Licensing: Must hold a valid Physical Therapist (PT) license in Arizona.
• Experience: Minimum of one year of home health experience.
