  • Responsibilities

    POSITION OVERVIEW - COMMUNITY HEALTH COACH

    As a Community Health Coach, you will conduct outreach, provide education and instruction, and gather and record information for community members that have tested positive for a contagious illness (the “primary”), or have been potentially exposed to a patient with a contagious illness (each a “contact”).  You will also support up to twenty Community Health Champions through oversight, mentoring, coaching and leading, and troubleshoot difficult issues with a primary and/or a contact.

    With respect to your contact tracing duties,   in an empathetic and culturally competent manner, you will:

    • Interview a primary and determine additional individuals who may be a “contact”;  
    • Discuss the basics of the illness, its potential signs and symptoms, and provide guidance on how the primary and the contacts can keep themselves and the community safe,   
    • Engage each primary and contact in ongoing monitoring activities, including the presence and severity of symptoms, and compliance with isolation guidance.  
    • Assess both primary and contacts preferred means of interacting with us during the monitoring period, and facilitate the best means of maintaining ongoing contact and information exchange (phone, personal visits, self-reporting, or other technology).   
    • Where appropriate, you will encourage both primary and contacts to self-report critical monitoring information using technology tools supplied by us or our partners   
    • Determine any further social or physical needs of the primary and any contact during the monitoring period and refer them to appropriate resources.   
    • Accurately and timely record all collected information into the information systems used by us, or those of our clients and partners, as required.  
    • Maintain confidentiality on all interactions, records and conversations.

    With respect to your Champion oversight duties, in a professional and supportive manner, you will:

    • Develop a positive team culture consistent with the company values among your Champions;
    • Monitor the performance of Champions, and provide constructive feedback as necessary;
    • Identify training needs for Champions, and implement group or individual development plans as needed.
    • Oversee productivity and compensation for Champions.
    • Serve as a company ambassador, as needed, with Champions, a primary or contact.  
    • Act as consultative resources for challenging problems involving a primary or a contact.

    POSITION QUALIFICATIONS - EDUCATION/EXPERIENCE

    You will only be qualified to perform this position if you meet the following minimum criteria, and will be at the head of the pack for consideration if you possess the preferred qualifications: 

    • Satisfactory completion of the Contact Tracing Training Program developed by Johns Hopkins University.  Please note that this training is available without cost at COVID-19 Contact Tracing Training 
    • Associate or Bachelor's Degree from an accredited college.  Concentration in life sciences, human services, communications or sociology preferred.
    • Experience managing a team; managing a remote team preferred. 
    • Public health expertise and/or experience preferred.
    • Prior experience in contract tracing, community outreach, and/or customer service preferred.
    • Experience navigating computer systems, utilizing databases and the internet for searches.
    • Ability to speak, read, and write English
    • Proficiency in languages other than English, desirable.

    POSITION QUALIFICATIONS - TECHNICAL/ENVIRONMENTAL

    You will need to make certain that your remote environment can support your duties.  This includes: 

    • An appropriate remote work environment free from interruptions and respectful of the sharing of confidential information (use of a headset is strongly encouraged).
    • Reliable broadband internet, and tools to access cloud-based applications where you will document your activities (may include tablet, laptop,  or another device).    Specifications for hardware and related testing will be completed prior to employment.
