Job Description

Community Outreach Specialists shall provide services to developmentally disabled or special need consumers in the following areas on an as needed basis according to individual needs.

Provides skill training in money management; psycho-social well being, bus/destination training, social etiquette and social appropriateness, health and personal hygiene, safety, utilization of community resources, special purchases and self advocacy.

Provide any assistance that may pertain to the overall well being of the Individual being served

Maintain current and accurate documentation on a per visit basis

Report Incidents and injuries in a prompt manner

Liaison to the community

Maintain good rapport and interaction with Participants as well as outside agencies and their personnel

Comply with Individual program goals and guidelines

Handle emergencies and crisis situations

This position may be subject to an On-Call status. Employees may be required to respond to participant and or company emergencies

Direct Support and Case Management positions available

Full Time and Part Time positions

Pay range from $13.50 -$19.00 per hour based on experience.
