Kenosha Estates, a skilled nursing facility in southern Wisconsin, is staffed by attentive, experienced caregivers 24/7 and offers a wide range of personalized care services. Kenosha Estates is looking for a Social Worker - Full Time.

    The right candidate for this role would be responsible for the following :

    Job Duties

    • Develops and implements social services programs to meet the needs of the residents in the facility and the needs of the community
    • Provides direct service and support to residents, such as conducting a needs evaluation and/or resolving complaints
    • Establishes and maintains relationships with other agencies and organizations in community to meet community needs and to ensure that services are not duplicated
    • Establishes and oversees administrative procedures to meet facility objectives for the social services program
    • Interviews residents and family members to compile information on social, educational, criminal, institutional, or drug history
    • Assesses residents social needs and conveys information to co-workers, medical records and care plans
    • Coordinates care plan meetings and counsels residents as needed
    • Reports/documents changes in resident social condition, or oversees the documentation by Social Services staff
    • Evaluates the work of staff and volunteers to ensure that programs are of appropriate quality and that resources are used effectively
    • Collaborates with management and coordinates the process to resolve resident complaints
    • Plans and collaborates with internal staff and outside agencies for programs, equipment and support services
    • Participates in the determination of organizational policies regarding such issues as participant eligibility, program requirements and program benefits
    • Prepares and maintains records and reports, such as grievance logs, Behavior Meeting Minutes,  personnel records and/or training manuals
    • Relates to residents, family members, public and professionals appropriately

    Job Requirements

    • Bachelor of Social Work Degree
    • Previous long-term care experience required
    • Ability to use desktop/notebook computers, Microsoft Office Suite
