    Keystone Advisors is looking for a Patient Navigator to join our team in Chicago, IL supporting one of our healthcare clients in Chicago, Illinois.

    The Patient Navigator performs a variety of coordinated and specialized tasks that support patients while they learn to self-navigate throughout the administrative operations of the Hospital ED department. Perform tasks such as directing patients to Provide quality service to all patients. Create a professional atmosphere in performing all job-related duties. Perform duties in a highly professional manner. Patient Navigators are committed to removing the client’s barriers to care by identifying critical resources for clients, helping them navigate through health care services and systems, and promoting client health Performs all job-related functions as required and without interruptions.

    JOB DUTIES:

    Establishes welcoming relationships with patients and serves as primary point of contact when patient visits the Emergency Department.

    Request interpreter Services (when needed or requested) for patients visiting the emergency room.

    Accompany Patients to through the ED/Trauma if needed or requested by patient or Clinical Staff.

    Performs visitors pass process authorizations remotely, utilizing computer on wheels (WOW).

    Greet and give Clinic directions to patients who arrive in the ED but are scheduled to be in another location for a clinic visit.

    Communicate with Care Team members to facilitate and Identify resources for patient to such as setting up transportation, and wheelchair access throughout the Emergency Department.

    Maintain strict confidentiality in accordance with Hospital policies.

    Attend in-service trainings, and meetings at the request of or with the approval of Site Manager/Supervisor

    Verifies and validates Visitors Pass process by Identifying that patient is in the ED/Trauma area and has no restrictions regarding visitation.

    Answers the telephone promptly and in a professional and courteous manner if located at Primary desk.

    Complies with all departmental and hospital policies/procedures and follows all safety standard and guidelines.

    Makes sure working relationships with peers are productive and without confrontation and interacts with patients, employees, and visitors in professional and courteous manner.

    During disasters, perform patient care tasks as requested by the Manager/Supervisors or AAIII assigned to the ED and Emergency Management personnel.

    Navigator will manage patients and visitors in the ED waiting room area by, monitoring the area, and reporting any incidents or concerns to the Care Team, Management staff and or CCH Police on duty when necessary.

    Flexible and adaptable in response to changes during any disaster within the Hospitals on Visitors Restrictions.

    Demonstrates knowledge of emergency procedures (i.e. fire/evacuation, internal and external disaster plans), and is aware of daily assignments.

    Demonstrates flexibility in changing workloads/assignments related to patient arrivals, discharges, census, and staff availability.

    Participate in special projects and other duties as assigned.

    MINIMUM QUALIFICATIONS:

    A High School Diploma or GED certificate is required.

    One (1) year clerical experience is required preferably direct public contact in a hospital setting.

    Type at a corrected rate of 25 words per minute

    PREFERRED QUALIFICATIONS

    Bilingual in English/Spanish

    Prior experience in a hospital or physician's office
