Our caregivers focus on each unique patient to deliver the appropriate care and emotional support to our patients and their families.

    As a Customer Service Representative, you will:

    • Make phone calls to clients to evaluate satisfaction level with attendant services.
    • Obtain information related to needs that can not be met by the Community Care Service Plan.
    • When additional needs of a non-emergency nature are identified obtain verbal consent.
    • Refer to the appropriate PHC Liaison for follow-up and referral.
    • Potential complaints or emergency situations are referred to the local Community Care office who serves the client for following up.
    • Document all calls and maintain per HIPAA requirements.
    • Other duties as assigned.

    Required Skills

    QUALIFICATIONS:

    • High School education required, at least two years of college preferred.
    • Good communication and problem solving skills required.
    • Ability to function in a high stress environment essential.
    • Bilingual skills preferred (Spanish).
