    As a, New Graduate RN, you will:

     

    • It is the expectation the New Graduate will follow his/her New Graduate orientation track as designed. This will ensure the observation and competency will be completed in the proper order.
    • Assess home health and/or hospice patients in stable and emergent situations to identify the physical, psychosocial and environmental needs as evidenced by documentation, clinical record, case conference, team report and evaluations.
    • Implement, develop, document and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
    • Obtain data on physical, psychological, social, spiritual and cultural factors that may influence patient/family health status and incorporate that data in to the Plan of Care (POC).
    • Assure continuity of quality patient care delivered with appropriate documentation. Maintain up-to-date patient records so that all visits, contacts, problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur.
    • Provide care utilizing infection control measures that protect both the staff and the Patient (OSHA).
    • Educate patients and/or their families regarding care, diet, medications, home safety, etc. Uses teaching methods appropriate to the age, condition, culture and assessed learning-readiness of the patient and/or family members.  Consistently teach and promote self-care practices. 
    • Assess and reassess pain. Utilize appropriate pain management techniques.  Educate the patient and family regarding pain management.
    • Arrange referrals to appropriate agencies when need exists.
    • Monitor assigned cases to ensure compliance with requirements of third party payer.
    • Provide effective communication and serves as a liaison to patients/family, team members, administration, physicians and other health care professionals.
    • Must attend interdisciplinary team meetings/case conferences, staff meetings and other meetings as appropriate.
    • Participate in Performance Improvement activities which may include clinical record review or participation in the PI Committee.
    • Documents appropriately on all patient visits. case conferences, physician orders, re-certifications,  discharges and care plan changes timely.. 
    • The following essential functions will not be assigned prior to after the completion of the first 90-120 days.
      • Supervise and provide clinical directions to the HHA/STNA/LVN to ensure quality and continuity of service provided.
      • Performs on-call responsibilities and provide on-call services to patients/families as assigned.
      • Provides case management under the supervision of the Clinical Manager in compliance with the state Registered Nurse Practice Act.
    • It is the expectation the skills checklist will be completed with the preceptor when the patient care opportunity arises. Any skills not signed off by the preceptor, will be the responsibility of the New Graduate and his/her Clinical Manager to prove competency prior to delivery of that specific care.
    • Perform other related duties as assigned or requested


    Required Skills
    • Graduate of an accredited school in Nursing or Nursing Diploma program.Bachelors of Science or greater in Nursing, preferred.
    • Current state license as a Registered Nurse.
    • Proof of current CPR and Hepatitis profile.
    • Current driver’s license. Must have reliable transportation and automobile insurance.
    • If permitted by state regulations, the New Graduate RN may be employed by the Home Health Agency when approved by the Regional Clinical and Operational Leadership.
    • Prior medical experience is preferred.
    • Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
    • Ability to communicate effectively with patients and their family members and at all levels of the organization.
    • Knowledge of medications and their correct administration.
    • Must have knowledge of venipuncture and IV care.
    • Is competent in (1) stage (as appropriate), measure and treat surgical wounds and decubiti, (2) administer inhalation therapy, (3) perform phlebotomy for outpatient lab testing, and (4) develop and initiate bowel and/or bladder training programs for the home care patient in addition to all routine medical/surgical nursing skills.
    • Is competent in (1) care and maintenance of indwelling catheters and drains, (2) safe use of all durable medical equipment routinely used in home setting.
    • Maintains current licensure certifications and meets mandatory continuing education requirements.
    • Ability to work flexible schedule and travel locally.
    • Ability to do heavy lifting, bending, pulling, pushing and standing. Prolonged standing and walking required.  Requires working under some stressful conditions to meet deadlines and patient needs and to meet patient/family individualized psycho social needs.
    • Must read, write and speak fluent English.
    • If the New Graduate RN is assigned a patient visit requiring a skill/procedure that he/she has not proved competency; the New Graduate RN has the right and the responsibility to inform his/her Preceptor or Clinical Manager and decline the patient visit.
