     Our Community Care division is actively seeking a LVN/LPN to join its team! As a LVN/LPN you will administer treatments under the guidance of a Registered Nurse.

     

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    Include the following; however other duties may be assigned:

    • Performs in-home client assessments, reports findings, and makes recommendations for changes in the plan of care.
    • Makes recommendations for improving care. Assist with obtaining annual and verbal orders as needed. Performs general health evaluations as assigned.
    • Provides skilled care to clients as authorized and completes on-site evaluation forms.
    • Notify supervisor and/or client within 24 hours of not being able to perform a visit for any reason.
    • Provides on-site evaluation for purposes of 10% sampling of client care and monthly evaluation of APC clients as assigned.
    • Completes all paper work properly and in a timely fashion. All paperwork should be turned into the office within 48-72 hours and should be legible and completed in accordance to agency policies and program rules and regulations.
    • Maintain confidentiality of client information.
    • Performs other duties as assigned.
    • Delivers services to clients only.
    • Comply with all program rules and regulations and agency policies and procedures.
    • Accept instructions and supervision, communicate appropriately and effectively with staff/supervisors/clients, performs under stress, practices self-control, maintains confidentiality at all times, exercises sound, professional judgment.

    Required Skills

    • Have experience in care of the elderly, disabled or infirm.
    • Must have knowledge of geriatric client care and be able to access support systems to meet client needs.
    • Must have ability to solve problems and work closely with client, family, caregivers and support agencies in maintaining quality care.
    • Must have reporting skills to meet State and AHCM requirements.
    • Current and active LVN/LPN license.
