    As an INTAKE TEAM LEADER, you will:

    • Ensure that all incoming referrals from referral sources/potential clients are responded to promptly. Provide information and determine the organization’s ability to provide requested services.
    • Identify alternative community or competitor services to the caller when solutions are not available.
    • Supervise the negotiation of rates and the frequency of visits with case managers. Ensure the credit application process is followed when necessary.
    • Ensure that the team makes an initial screening, both clinical and payer specific, for appropriateness of homecare services.
    • Ensure the recording and tracking of outcomes related to all inquiry/referral calls received within the market (e.g., conversion rates, reasons for non-conversion within service delivery units, etc.) by reviewing weekly tracking logs.
    • Oversee and ensure that all potential payer sources are identified and ensure correct determination of primary payer by reviewing documents created within the department. Provide ongoing staff development of team members regarding these issues.

    Required Skills

    QUALIFICATIONS

    • Associate’s Degree or the equivalent
    • Minimum of five years clinical experience in the area of specialty
    • Two years in a supervisory role generally required
    • Acute/Homecare experience within two years and a broad knowledge of case management, governmental home health agency regulations, and Medicare
    • Excellent organizational, problem-solving, and communications skills
    • Basic proficiency with computer database entry, Internet, and basic software programs, e.g., Microsoft Office
