Our caregivers focus on each unique patient to deliver the appropriate care and emotional support to our patients and their families.

     

    As a CASE MANAGER, you will:

    1. Function as the facilitator of the Interdisciplinary Team Meeting and in conjunction with the member, informal/formal support and available resources, assesses the member’s needs for in-home services.
    2. Develop a comprehensive service plan for the Advantage member. The service plan is developed to utilize the member’s strengths, supports and available resources and is outcome focused.
    3. Involve the member in implementation of the service plan by reviewing the services and receiving signed authorization.
    4. Coordinate with the Case Manager Supervisor and with DHS to complete the service plan authorization process.
    5. Coordinate all aspects of service delivery, ensuring that all services are delivered according to the service plan.
    6. Complete on-going monitoring with the member. The member is monitored for appropriateness of services, for health and safety issues, for re-evaluation of the service plan.
    7. Complete the Universal Comprehensive Assessment Tool (UCAT) as needed according to the Advantage Program Policies and Procedures.
    8. Communicate with the Director of Member Care, Branch Manager, Director of Nursing, the Case Manager Supervisor and Coordinators as necessary, regarding member issues or information.
    9. Documents all member contacts and maintains accurate and complete member files.
    10. Participate in all appropriate agency staff meetings and inservices.
    11. Maintain confidentiality of member, attendant and staff information.
    12. Adhere to and participates in Company’s mandatory HIPAA privacy program / practices, Business Ethics and Compliance programs / practices, and all Company policies and procedures.
    13. Participate in special projects and performs other duties as assigned.

    Required Skills

    QUALIFICATIONS:

    • Bachelor's Degree or RN or LPN in lieu of Bachelor's Degree.
    • One year paid professional experience with the aging or disabled population obtained before or after receipt of degree. (Paid professional experience may include, but is not limited to: CNA, CMA, CHHA and PCA experience ).
    • Bilingual (Spanish) preferred.
    • A minimum of one week of orientation to the agencies policies and procedures to include shadowing a certified Case Manager in the field.
