Our caregivers focus on each unique patient to deliver the appropriate care and emotional support to our patients and their families.

    As a HOME HEALTH DIETICIAN, you will:

    • Interview patients and assess/evaluate their nutritional status.
    • Determine patients' dietary needs and advise patients' physicians and/or nursing personnel of needs.
    • Confer with patients' physicians to establish dietary treatments and requirements.
    • Prepare and calculate therapeutic diets to meet the specific requirements of patients.
    • Provide guidance, assistance, and/or explanations to patients and family members to help them understand, accept, and follow dietary modifications ordered by the physician. Instruct patients and family members on dietary restrictions and drug/nutrient interactions as appropriate.
    • Observe patients' responses to dietary treatment, note changes in their condition, and advise physicians, Nurse Supervisors and/or Clinical Coordinators. Revise nutritional plan as appropriate based on patients' condition and physician’s orders.
    • Participate in case conferences, and act as a resource to other health team members as requested.
    • Maintain and submit paperwork/documentation as required by the Company and/or facility.
    • Ensure the confidentiality of patients' files and documentation.

    Required Skills

    QUALIFICATIONS

    • Degree in Nutrition or the equivalent
    • Minimum of six months related experience
    • Current Registered Dietician licensure/certification as necessary in the state(s) of practice
    • Good organization and communication skills
