    Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. It includes eight primary care sites, 40 school-based health centers and dental clinics, four day care centers, the nation's largest dental residency program, and New York State's largest behavioral health program.     POSITION SUMMARY: We have an exciting opportunity to join our team as a Coord, School Health. In this role, the successful candidate School Health Program Coordinator will report directly to the Administrative Director, and is responsible to proactively support the School-Based Health Program in tasks related to outreach and enrollment, operational management of schools sites, quarterly reports, quality improvement, and certain CQI and compliance responsibilities.

    JOB RESPONSIBILITIES:

    • Supports implementation of the outreach strategy of the School-Based Health Program, including development, coordination, and monitoring of key deliverables in accordance with departmental and organizational goals and timetables.
    • Required to have consistent presence at school sites, and liaise with parent coordinators with a focus on maximizing enrollment and utilization potential. Coordinates and participates in outreach efforts with school sites to ensure all school sites meet the minimum requirement of 75% enrollment.
    • Maintains outreach calendar.
    • Informs leadership of any situations that may impact the SBHC's relationship with patients, families, community members, or school staff.
    • Oversees the physical environment of school sites, interfacing with other departments including IT, telecommunications, and facilities; escalates repair issues and/or the need for replacement supplies.
    • Solves day-to-day operational problems of school sites (e.g., staffing, interactions within other areas and departments).
    • Insures quality and performance of operational functions to achieve desired outcomes as well as meeting all compliance and regulatory requirements.
    • Primarily responsible for Family Planning Benefit Program (FPBP) enrollment efforts and forms submission.
    • Assists in the preparation of quarterly and year-end reports by use of OSCR and Epic reporting tools.
    • Attends and presents at monthly program meetings, and assists with preparation of agenda and planning.
    • Assists with implementation of new sites
    • Assists with review of Sunrise laboratory billing to verify accuracy
    • Assists in the coordination of yearly Point of Care Testing Validations for all Medical Associates (MAs).
    • Participates in monthly Environment of Care meetings.
    • Assists in the coordination of continuing quality improvement (CQI).
    • Sign-off on purchase requests not to exceed $300.00.
    • Assists with provider schedule maintenance.
    • Attends and actively participates in the planning of the yearly school health Advocacy Day trip to Albany.
    • Serves as a back-up to Administrative Associate in maintaining School Health employee timecards; processes payroll at the end of each pay period.
    • Coordinates interviews with potential candidates.

    MINIMUM QUALIFICATIONS:

    • To qualify you must have a Bachelor's Degree preferred. Minimum of an Associate's Degree or suitable combination equitable to some college and experience.

    • Demonstrate the ability to proactively manage multiple deadlines.
    • Detail-oriented, organized, and resourceful.
    • Able to work independently with close attention to detail.
    • Prior experience with enrollment and/or outreach.
    • Prior experience with parent communication and engagement.
    • Demonstrated ability to conduct information sessions for internal and public audience.
    • Must be able to effectively communicate with all levels of the organization.
    • Proficiency with Excel and management of large data sets.
