    JOB TITLE: HOUSING OUTREACH AND STABILIZATION COORDINATOR (PART-TIME) DIVISION/UNIT: HOUSING AND COMMUNITY SERVICES LOCATION: BREMERTON, WA SALARY:   $17.90 PER HOUR (NON-EXEMPT) CLOSES:    OPEN UNTIL FILLED NOTE: A RESUME AND COVER LETTER ARE REQUIRED ALONG WITH A COMPLETED APPLICATION. ANY INCOMPLETE APPLICATIONS WILL NOT BE CONSIDERED.  JOB SUMMARY:

    The Housing Outreach and Stabilization Coordinator will be a key part of the multi-agency Kitsap Connect team, which also includes Kitsap Mental Health Services and the Kitsap Public Health District.  The purpose of Kitsap Connect is to work directly with the most vulnerable persons in our community who are also high utilizers of costly services, and provide them tailored, intensive outreach and coordinated care focused specifically on stabilizing housing, accessing primary care, mental health, and/or chemical dependency services.  This person participates on the crisis response and coordination team and is responsible to help lead outreach and engagement efforts for referred clients facing chronic homelessness or extreme housing instability due to mental health, chronic health, or chemical dependency issues.  Working in close coordination with the Housing Solutions Center, this position assists client's secure stable, appropriate housing, and maintaining that housing over time.

    This position will be a KCR employee and also have programmatic reporting responsibility to the Kitsap Connect Program Coordinator, who is an employee of the Kitsap Public Health District. Responsibility for supervision and performance evaluations will be shared between the HSC Supervisor and the Kitsap Connect Coordinator. The Health District will provide overall administrative leadership for this collaborative team. Position will have multiple worksites; primary worksite is at KCR – may also work at Salvation Army/other sites, as program needs require.

    MINIMUM QUALIFICATIONS

    Education and/or Experience:

    • Requires a High School Diploma or GED plus 3.5 years' experience in related field or AA degree plus 1.5 years of experience.  Substantial experience may be substituted for education requirements.

    Knowledge, Skills and Abilities:

    • Experience with working with the homeless population or providing housing case management to homeless persons is a plus.
    • Knowledge of the needs of our community's most vulnerable residents and a working knowledge of available and appropriate community resources.
    • Excellent communication skills
    • Strong computer skills and be comfortable working with spreadsheets and databases.
    • Must be a self-starter and able to work independently with limited supervision.
    • Must be willing to complete Trauma Informed Care, Motivational Interviewing, and Stages of Change Theory training within first six months.
    • Must work scheduled hours; (program related exceptions require prior approval of HSC Supervisor)
    • Ability to pass a Washington State Criminal Background Check

    License(s) and/or Certifications:

    • Valid Washington State Driver's License
    • Current auto liability insurance and reliable transportation
    • Family Development Specialist Certification

    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    1. Reports administratively to the Housing Solutions Center Supervisor and accepts assignments from the Kitsap Connect Program Coordinator.
    2. Follows up with individuals referred to this program to determine their willingness to participate in the Kitsap Connect program. Provides ongoing outreach and assessment services for participating persons.  These services will be offered where the person currently resides, which may be outdoors. 
    3. Identifies a person's or household's primary housing barriers (employment, substance abuse, mental health, education, domestic violence, health), develops the recommended housing strategy integrating Housing First approaches, and participates in care coordination conferences with the Kitsap Connect Public Health Nurse, the Kitsap Connect Behavioral Health professional and other community providers which may include healthcare, law enforcement, criminal justice, social services other housing providers, and others.
    4. Partners with HSC navigators to identify permanent housing or housing stability options for which they may be eligible, work with potential landlords to overcome barriers, and coordinate referrals to community services that may extend beyond housing.
    5. Cultivates relationships with program participants and uses tact, discretion, respect and courtesy as a means to gain trust and encourage the cooperation of others. Maintains effective working relationships with co-workers, representatives of other agencies, and program participants.
    6. Documents the housing outreach processes and procedures.
    7. Tracks client records in an Electronic Medical Record and maintains complete and accurate client files and quality data into the HMIS data system.
    8. Assists the team in tracking and collecting and compiling client statistics and in the completion of program reports.
    9. Develops and expands community partnerships in consultation with the Kitsap Connect Program Coordinator.
    10. Follows Agency Code of Ethics regarding safeguarding the confidentiality of client information.
    11. Follows agency policy related to Employee Safety.
    12. Accepts other duties as assigned.
