FOUNTAIN OF JOY DDD group homes located in Mesa are open 24 hours a day, 7 days a week, and 365 days a year. As a Lead staff, your responsibility is to:
Provide direction to direct support staff during the shift
Assign tasks such as medication passes, hygiene support, documentation, meal prep, and activities.
Serve as the point of contact for emergencies, incidents, and family or supervisor communication.
Mentor and train new staff members.
Ensure each resident’s needs are met according to their Individual Support Plan (ISP) or care plan.
Support residents with daily living skills (ADLs): bathing, grooming, meals, cleaning, mobility, etc.
Facilitate community outings, appointments, programs, and skill-building activities.
Advocate for residents’ rights, preferences, and safety.
Administer medications (if certified) and ensure accurate documentation.
Monitor residents for health changes and communicate any concerns to supervisors or nurses.
Coordinate doctor appointments and follow-up care.
Complete and review shift notes, incident reports, MARs, and daily logs.
Ensure staff follow agency policies, licensing regulations, and safety protocols.
Maintain confidentiality and uphold professional standards.
Oversee household routines: cleaning, meal planning, grocery shopping, and supply inventory.
Ensure the home environment is safe, clean, and comfortable.
Assist with budgeting or petty cash, depending on agency policy.
Communicate effectively with supervisors, families, case managers, and healthcare providers.
Address conflicts between staff or residents professionally.
Report concerns and help implement behavior support plans or crisis interventions.
Lead responses to medical or behavioral emergencies.
Ensure all staff follow crisis procedures.
Complete incident reporting accurately and in a timely manner.
Responsibilities:
Provide assistance, monitoring, and training in the areas of self-care, self-direction, independence, mobility, and learning, as outlined in the Individual Service Plan (ISP), to enhance each individual's skill development, health, freedom, and personal dignity.
Maintain the cleanliness of the environment.
Possess effective communication skills.
Assist in behavior management and crisis intervention.
Administer healthcare, including medication, nutrition, allergies, and meal planning.
Assist with safety skills, such as basic first aid, fire drills, and handling emergency situations.
Maintain confidentiality and privacy.
Meet the requirements of federal, state, and local organizations, and maintain meticulous record-keeping.
Uphold optimal attendance and reliability.
Exercise good judgment and the ability to evaluate and anticipate consequences.
Fulfill any other duties as assigned.
As a Caregiver / Direct Care Professional:
You are a mandatory reporter. If you suspect any form of abuse or neglect, you must promptly contact the appropriate authorities, DCS/APS, and your direct supervisor.
Complete incident reports as directed.
Maintain current qualifications and training.
Basic Qualifications:
18 years of age or older.
1 year of caregiving experience is preferred.
High school diploma or equivalent is preferred.
A valid driver's license is preferred.
CPR/First Aid certified.

Prevention & Support.
Ability to obtain and maintain a valid driver's license.
Proficiency in the English language, both written and verbal.
The ability to interpret a variety of instructions furnished in written, oral, or scheduled form.
Must be capable of frequently lifting/moving up to 10 pounds and occasionally lifting and moving more than 100 pounds.
Completion of training programs and successful passage of associated tests.
Ability to obtain a Level 1 Arizona Fingerprint Clearance Card and pass a background check as required by state and federal regulations.
