About the Role:

Position Summary:
The Licensed Practical Nurse (LPN) provides direct nursing care and health-related support to participants under the supervision of a Registered Nurse (RN) or Director of Nursing. The LPN is responsible for monitoring health status, administering medications and treatments, and supporting the implementation of individualized care plans to ensure the safety, well-being, and quality of life of participants.
This role works closely with interdisciplinary team members to deliver person-centered care, respond to medical needs, and promote positive health outcomes. The LPN assists in maintaining accurate clinical documentation, observing and reporting changes in condition, and ensuring all services are delivered in accordance with applicable federal, state, and local regulations.
The LPN ensures compliance with Ohio nursing practice standards, including those established by the Ohio Board of Nursing, as well as applicable requirements from the Ohio Department of Aging, and Medicaid programs, as relevant to the service settings.
Duties and Responsibilities:
Supporting the Registered Nursing during on-site medical or health care emergencies; dispensing medications and perform treatments according to physician orders; supervising participants at lunch to monitor eating patterns; provide direction to nursing assistants and personal care aides regarding participant safety issues; participating in therapeutic recreation activities as needed; and providing written documentation regarding the health status of participants, as prompted by their interdisciplinary care plan.
Responsible for performing clinical services that will support the overall health and wellness of participants and support their interdisciplinary care plan. This will include new participant intakes; conducting cognitive, social, nutritional and functional assessments; manage illnesses or health issues, including verbal and written contact with physicians and other health care professionals.
Participates in scheduled care plan meetings for the participants.
Responsible for the overall day-to-day health and wellness management of the participants attending the adult day center. This includes overall compliance oversight of all aspects of medical paperwork, medication management, daily and monthly wellness visits, and medical record documentation, coordination of care with participant responsible party, various health professionals (physician, therapy services, hospice, etc.), annual physicals and health assessments, and general monitoring of the health status of the participants.
Works closely with the team of the center to ensure that participants’ health and wellness needs are being met through the programs and care given at the Center and documented on the care plan.
Plays a vital role in the education and training of staff in areas of assistance in all ADLs, infection control, non-pharmacological behavioral interventions, proper body mechanics, etc.
Assists in providing trainings in the community as well as providing care consultations to the families, caregivers, and the community at large.
Supervises Direct Support Professionals.
Reports any changes in participants’ conditions to the Program Director.
May aid participants with activities of daily living.
Employee must maintain effective communication with all staff, participants, caregivers and/or families.
Must maintain participant confidentiality at all times.
Maintain a professional attitude and adherence to state licensing policies and procedures.
Completes continuing education as required and maintains any certifications/licensure essential to the position.

Required Qualifications:
Must be a graduate of an accredited nursing institution and be licensed as a Practical Nurse (LP/LVN).
Have a current licensure for the state the center is located in.
Must have effective written and verbal communication skills.
Effective communication with all staff, participants, caregivers and/or families.
Must have good organizational and time management skills are essential.
Must have experience in Electronic Medical Records (EMR) platform.
Preferred Qualifications:
Minimum of 1 year of experience in long-term care (residential, adult day services, community-based care) or dementia care.
Physical Demands:
Ability to sit, stand, walk, bend and squat for prolonged periods; ability to push and pull objects and lift and carry up to 20 pounds, unassisted, on a frequent basis and occasionally up to 50 pounds. Full use of gross and fine motor skills of the upper and lower extremities (i.e., writing in charts, assisting with toileting and incontinence care, assisting with meals, activities, etc.)
Working Conditions:
Primarily inside in well-lit, well-ventilated areas up to 8 hours a day.
