    Job Description

    ABOUT US: Our client is a community-based health plan dedicated to improving the health and well-being of the residents of the Bay Area. Working in partnership with providers and community organizations, they serve our neighbors through our Medi-Cal and Cal MediConnect health insurance plans JOB SUMMARY:

    • Functions as a member of the care team by providing case management and coordination for clients
    • Coordinate and assist in client scheduling
    • Coordinate medical follow-up
    • Under direction of medical staff, facilitate specialty referrals, including making necessary appointments and client contact
    • Contact referral resources to ensure necessary reports are received at the center
    • Handle client correspondence, as necessary
    • Maintain client records and provide record transfers
    • Maintain accurate statistical information including necessary logs and documentation to track client compliance with referrals and prepare reports
    • Coordinate pre-visit planning
    • Oversee outreach to new and existing patients
    • Organize daily huddles and participate in team meetings
    • Assist clinician during procedures when necessary
    • Contribute to achieving health center productivity goals
    • Uses clear and culturally sensitive language when communication with patients, staff, and outside providers

    QUALIFICATIONS:

    • Bachelor’s Degree in Health Sciences, Public Health, or Health Care or related field, or equivalent experience, training or coursework
    • Minimum one year of experience working in a healthcare setting supporting QI and/or other related healthcare programs
    • Prior experience with managed care plans, Medi-Cal and Medicare programs, and working with undeserved populations
    • Ability to pay attention to detail and work accurately
    • Ability to form positive, professional relationships with internal and external stakeholders
    • Ability to analyze data
    • Critical thinking with the ability to look for gaps and ensure contractual requirements are being met by department programs
