    Job Description: LMSW

    Responsible To: Mental Health Manager

    Position Summary: Under the direction of the Director of Psychiatry and Mental Health, provides group, individual and family therapy sessions to people struggling with mental health, alcohol or drug problems.

    Qualifications: Qualified candidates MUST have a valid and active License for the state of Texas. Bachelor’s degree/Doctorate in Psychology, Sociology, Counseling, Social Work and a Minimum of 2 years related work experience. Must have a Valid Texas driver’s license and proof of automobile insurance. Must possess a working knowledge of techniques to manage symptoms of mental illness and prevent relapse. Must demonstrate familiarity with area-wide community resources relevant to mental health and basic needs-related services. Must demonstrate ability to provide hopeful, supportive and effective rehabilitation.

    Significant Duties and Functions

    · Provides direct care in determining patient’s status and issues by interviewing them, obtaining personal information and medical history.

    · Conducting assessments and evaluations of clients to determine their mental health and social service needs

    · Developing individualized treatment plans and providing therapy to clients in a one-on-one or group setting

    · Collaborating with interdisciplinary teams to coordinate and provide comprehensive care to clients

    · Maintaining accurate and confidential client records and documenting progress towards treatment goals

    · Staying current with best practices, laws, and regulations in the field of social work and mental health

    · Actively participating in supervision, team meetings, and professional development opportunities

    · Modifies treatments by maintaining case history and progress notes while observing their patients.

    · Restores patients to productive roles by educating them about available community-based organizations,

    and social and employment services.

    · Follows HIPAA guidelines

    · Follows infection control policies and procedures

    · Complies with JACHO, PCMH and HRSA Standards

    · Participates in outreach activities and other company events

    · Participates in QI/QA Activities and Initiatives

    · Reports on demographic, SOGI, SDoH and other pertinent data

    · Provides services in the community using personal vehicle

    · Ability to work in a dynamic team setting with medical and clinical professionals

    · Develop treatment plans related to rehab services goals

    · Provide services in the clinic as needed

    · Provide individual facilitation of skills-building and psycho-educational groups

    · Caseload will consist of adult clients and children/adolescents

    · Share 24-hour on-call responsibilities including face-to-face interventions with other

    members of the team

    · Coordinate clients’ individual recovery needs internally and externally in the community

    · Follow all ADAPT policies and procedures and exercise good judgment and initiative in

    keeping with the varied and ever-changing needs of clients served;

    · Work schedule is highly dependent on client needs and can be ever-changing

    · Follow infection disease control Policies and Procedures

    · Maintains client confidentiality and adherence to HIPAA requirements at all times

    · Other duties as assigned.

    · Experience working in a mental health or social services setting

    · Specialization in a specific population or issue area, such as trauma, addiction, or geriatrics

    · Bilingual in a language commonly spoken by the client population

    · Experience working with electronic health records and documentation systems

    · Master's degree in social work from an accredited program

    · Current and valid license as a LMSW in Texas

    · Strong knowledge of psychotherapy theories and techniques

    · Excellent verbal and written communication skills

    · Ability to work independently and as part of a team

    · Commitment to providing culturally competent care to a diverse population
