We are seeking a Licensed Practical Nurse (LPN) to join our team and work closely with our case management staff to support coordinated, patient-centered care.

Key Responsibilities:

Assist with patient assessments and monitoring

Collaborate with case managers and care team members

Perform medication set-ups (med-set ups) and administer medications as ordered

Document patient status and report changes

Educate patients and caregivers on care plans and medications

Qualifications:

Active LPN license in good standing with the Indiana State Board of Nursing

Compliance with Indiana licensing requirements (IPLA), including renewal and scope of practice

Strong communication, organization, and teamwork skills

Experience in home health or case management preferred
