Aviva Senior Living is seeking a compassionate and skilled Licensed Practical Nurse (LPN/LVN) to join our senior living healthcare team in Valparaiso, Indiana. This is an excellent opportunity for an experienced LPN looking for a full-time or part-time evening shift nursing position in a senior living, assisted living, or memory care community.

The LPN will provide high-quality direct resident care, administer medications and treatments, monitor vital signs, assist with resident assessments and care plans, document clinical information, and oversee care provided by caregivers and other team members.

LPN Evening Shift – 2 PM to 10 PM

We are currently hiring a Licensed Practical Nurse (LPN) for our 2:00 PM – 10:00 PM evening shift.

This position may be full-time or part-time. Shift assignments may change during employment based on community and resident care needs, with appropriate notice.

Licensed Practical Nurse (LPN) Responsibilities

The Licensed Practical Nurse will be responsible for providing safe, compassionate, and individualized care to senior living residents.

Primary LPN Duties:

• Administer medications and treatments as prescribed by physicians and registered nurses
• Perform medication administration and medication management according to applicable policies and regulations
• Monitor and document resident vital signs, including blood pressure, temperature, pulse, respiration, and other clinical measurements
• Assist with resident assessments and individualized care plans
• Provide direct resident care and assistance with activities of daily living (ADLs)
• Assist residents with bathing, dressing, grooming, feeding, mobility, and other personal care needs
• Monitor residents for changes in condition and report concerns to appropriate healthcare professionals
• Assist with diagnostic tests, procedures, and treatments as needed
• Maintain accurate and timely clinical documentation and medical records
• Communicate with residents and families regarding care, condition, needs, and care plans
• Collaborate with physicians, registered nurses, caregivers, medication aides, and other healthcare professionals
• Provide direction and oversight to caregivers and other resident care team members
• Follow established nursing procedures, safety protocols, infection control practices, and community policies
• Promote resident dignity, independence, comfort, safety, and quality of life
• Respond appropriately to resident care needs and changes in resident condition
• Maintain confidentiality of resident and healthcare information
• Support a positive, respectful, and compassionate senior living environment

LPN Qualifications and Requirements

• Active Licensed Practical Nurse (LPN) license valid in the state where the position is located
• Licensed Vocational Nurse (LVN) credentials may be considered where applicable
• Minimum of 1 year of LPN/LVN nursing experience
• Experience in senior living, assisted living, memory care, long-term care, skilled nursing, rehabilitation, or healthcare is beneficial
• Strong knowledge of medication administration and resident care practices
• Excellent communication and interpersonal skills
• Strong clinical judgment and attention to detail
• Ability to recognize and communicate changes in resident condition
• Ability to work independently and collaboratively as part of a healthcare team
• Strong organizational and time-management skills
• Ability to prioritize multiple resident care needs in a fast-paced environment
• Basic computer and electronic documentation skills
• Ability to maintain accurate resident records and nursing documentation
• Compassionate, dependable, professional, and resident-focused approach to care
• Ability to treat residents, families, caregivers, nurses, physicians, and team members with respect and compassion
