Responsibilities:
Provide direct patient care, including administering medications and monitoring vital signs.
Assist with daily living activities and personal care for hospice patients.
Collaborate with the healthcare team to develop and implement individualized care plans.
Educate patients and families on care options and end-of-life issues.
Document patient progress and communicate changes to the nursing staff and physicians.
Participate in team meetings and contribute to quality improvement initiatives.
Ensure compliance with all regulatory and safety standards in hospice care.
Provide emotional support to patients and families during challenging times.
Requirements:
Current LPN license in the state of Georgia.
Minimum of 1 year experience in hospice or palliative care preferred.
Strong clinical assessment and communication skills.
Ability to work independently and as part of a multidisciplinary team.
Compassionate demeanor with a commitment to patient-centered care.
Basic Life Support (BLS) certification required.
Flexibility to work various shifts, including weekends and holidays.
Reliable transportation and willingness to travel to patient locations.
