    Position Overview We are seeking a LPN with experience or strong interest in hospice and palliative care to provide compassionate, patient-centered medical care for individuals at the end of life.

    This role blends clinical excellence with relational presence. You will support symptom management, guide complex care decisions, and collaborate closely with an interdisciplinary hospice team to ensure care aligns with each patient’s values and goals.

    What You’ll Do

    Provide direct nursing care to hospice patients in homes, assisted living, or facility setting under supervision of the RN case manager and DON

    Perform and document skilled nursing tasks

    Monitor patient condition and report changes promptly to RN case manager

    Collaborate with physicians, RNs, social workers, and chaplains

    Ensure compliance with hospice regulations and documentation standards

    Participate in on-call rotation as assigned

    Qualifications

    Active Licensed Practical Nurse license in Oregon

    Experience in hospice, palliative care, geriatrics, or complex care preferred

    Strong communication skills and comfort with end-of-life discussions

    Knowledge of hospice regulations and interdisciplinary care models

    Ability to work independently while collaborating closely with a care team
