Abound Home Health Care is seeking a reliable and compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes. The LPN will work under the supervision of an RN and follow each patient’s individualized plan of care.

Responsibilities

• Provide skilled nursing care in the patient’s home • Monitor vital signs and changes in patient condition • Administer medications and treatments as ordered • Perform wound care and other skilled nursing procedures as appropriate • Provide patient and caregiver education • Complete accurate and timely visit documentation • Communicate changes in condition to the supervising RN and provider • Follow agency policies, infection control practices, and the established plan of care
