About the Role:
We Care For You Senior Services is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our growing team in Southbury, CT. Responsibilities:
Provide direct patient care to senior residents in accordance with established care plans and clinical protocols
Administer medications and treatments as prescribed, ensuring accurate documentation
Monitor and record patient vital signs, symptoms, and changes in condition
Collaborate with RNs, physicians, and interdisciplinary care teams to coordinate resident care
Assist with wound care, catheter care, and other skilled nursing procedures
Communicate effectively with residents, families, and caregivers regarding care plans and health updates
Maintain accurate and timely medical records in compliance with state and federal regulations
Requirements:
Active LPN license in the state of Connecticut (required)
Previous experience in geriatric, long-term care, or senior services settings preferred
Strong knowledge of medication administration and clinical nursing procedures
Excellent communication, organizational, and critical thinking skills
Compassionate, patient-centered approach to elder care
Ability to work independently as well as collaboratively within a care team
CPR/BLS certification preferred
