    Job Summary

    As the LVN, Clinical Support, your primary function is to coordinate branch operations under the supervision of the Managing Director. You will manage scheduling for field staff based on triage needs, facilitate Interdisciplinary Team (IDT) meetings, and run essential reports to ensure the branch remains in full regulatory compliance.

    Key Responsibilities

    • Clinical Coordination: Schedule patient visits per RN triage or manager instructions and assist field nurses with prioritizing their daily schedules.
    • Communication & Support: Act as a point of contact for families, providing phone information, education, and addressing concerns while ensuring all calls are documented.
    • IDT Management: Prepare paperwork and coordinate notes for IDT meetings to ensure patient findings and condition changes are communicated appropriately.
    • Compliance & Documentation: Run branch reports to ensure compliance and ensure all physician orders are signed and clinical documentation is accurate.
    • Collaboration: Work closely with the marketing team, referring agencies, and the clinical team to maintain high-quality care standards.

    Qualifications

    • Education: Graduate of an accredited school of vocational nursing.
    • Licensure: Current State license as a Licensed Vocational Nurse (LVN).
    • Experience: Minimum of two (2) years of experience as an LVN in a clinical care setting; experience in hospice or home health is preferred.
    • Skills: Strong nursing skills, excellent interpersonal and communication abilities, and proficient computer and organizational skills.
    • Requirements: Valid State Driver’s License, reliable transportation, and current auto liability insurance.
      • Must pass a criminal background check.
      • Proof of TB testing and Hepatitis acceptance or declination.

    Working Conditions

    This role involves a blend of remote and office-based work. It requires long periods of sitting, significant computer work, and frequent telephone interaction. Candidates must be able to make quick decisions and handle stressful conditions to meet deadlines and patient needs.
