About the Role:
ComForCare Home Health Care (North San Diego) is looking for a compassionate and skilled LVN to join our dedicated home health team in San Diego, CA. This is a rewarding opportunity to make a real difference in patients' lives by delivering personalized, one-on-one care right in the comfort of their homes. Responsibilities:
Provide skilled nursing care to home health patients in accordance with physician-approved care plans
Administer medications, wound care, injections, and other LVN-scope treatments
Monitor and document patient health status, vital signs, and progress toward care goals
Communicate effectively with RNs, physicians, and interdisciplinary care team members
Educate patients and family members on disease management, medications, and home safety
Maintain accurate and timely clinical documentation in compliance with home health regulations
Build trusting relationships with patients to support continuity of care and positive outcomes
Requirements:
Valid California LVN license in good standing
Prior home health, skilled nursing, or clinical experience preferred
Current CPR/BLS certification
Valid driver's license, reliable transportation, and active auto insurance
Strong clinical assessment and critical thinking skills
Excellent communication, documentation, and time management abilities
Compassionate, self-motivated, and comfortable working independently in the field
