Responsibilities:

Provide direct patient care, including administering medications and monitoring vital signs.

Assist with daily living activities and ensure patient comfort and safety.

Collaborate with healthcare teams to develop and implement personalized care plans.

Educate patients and families on health management and wellness strategies.

Document patient progress and report any changes in condition to the supervising nurse.

Maintain compliance with healthcare regulations and company policies.

Participate in ongoing training and professional development opportunities.

Build strong relationships with patients and their families to foster trust and support.

Requirements:

Current LVN license in the state of California.

Minimum of 1 year of nursing experience, preferably in home health or hospice care.

Strong clinical assessment skills and attention to detail.

Excellent communication and interpersonal skills.

Ability to work independently and manage time effectively.

Compassionate demeanor with a commitment to patient-centered care.

Basic Life Support (BLS) certification required.

Reliable transportation and valid driver’s license.
