** Job Summary**
As a Licensed Vocational Nurse, your primary mission is to provide skilled nursing care under the supervision of a Registered Nurse (RN). You will coordinate care with an interdisciplinary team, patients, families, and referring agencies to ensure quality of care and adherence to the plan of care.

Qualifications
Education: Must be a graduate of an accredited school of vocational nursing.
Licensure: Current State license as a Licensed Vocational Nurse.
Experience: Two years' experience as an LVN in a clinical care setting is required ; experience in hospice or home health is preferred.
Skills: Nursing skills as defined by generally accepted standards of practice and good interpersonal skills.
Requirements: * Current State Driver's License, reliable transportation, and valid auto liability insurance.
Working Conditions
This position involves traveling locally to visit patients in their homes or facilities. You should be comfortable working in various conditions, including possible exposure to blood, bodily fluids, infectious diseases, and unpleasant weather. The role requires a flexible schedule, PRN emergency calls, and physical effort including prolonged standing, walking, and the ability to lift up to 50 lbs and move patients. You must be able to make quick decisions under stressful conditions to meet patient needs.
