Job Summary
We are looking for a compassionate and detail-oriented Licensed Vocational Nurse (LVN) to join our Adult Day Care team. In this role, you will provide essential nursing care to our participants, ensuring their health and safety throughout the day. You will work under the supervision of a Registered Nurse (RN) to implement care plans, administer medications, and monitor the well-being of seniors and adults with disabilities in a vibrant, community-focused environment.
Essential Duties & Responsibilities
Participant Care: Conduct morning health screenings, monitor vital signs, and perform routine daily assessments and checking participants monthly weights.
Medication Management: Safely administer oral and hypodermic medications and maintain accurate MAR (Medication Administration Record) documentation.
Clinical Support: Perform glucose monitoring, insulin administration, wound care/dressing changes, colostomy bag change and catheter care as needed.
Emergency Response: Provide first aid and CPR in emergency situations; coordinate with emergency services and families when necessary.
Documentation: Maintain detailed daily nursing notes and assist the RN with quarterly or semi-annual participant re-evaluations entry.
Collaboration: Work with the multidisciplinary team (Social Workers, Activities Directors) to ensure care plans align with participant needs.
Engagement: Assist participants with Activities of Daily Living (ADLs) during the day, including meal assistance and mobility support.
Qualifications
License: Current, unrestricted California LVN license.
Certification: Valid CPR/BLS and first aide certification.
Experience: Minimum 3 year of nursing experience (Geriatric, Home Health, or Rehab experience preferred).
Skills: Strong interpersonal skills, ability to remain calm in a fast-paced environment, and proficiency in electronic charting.
Physical: Ability to lift/transfer participants (up to 50 lbs) and stand for extended periods.
