We are seeking a motivated and compassionate Licensed Vocational Nurse (LVN) to join our dynamic wound care team.     Job Summary

    As a Field Wound Care LVN, you will play an essential role in the assessment, treatment, and ongoing management of patients with acute and chronic wounds. You will provide hands-on wound care, including dressing changes, cleansing techniques, and Ultramist therapy, while traveling to patient homes, skilled nursing facilities, assisted living communities, and clinic settings throughout Riverside and San Diego Counties. Your clinical skill, reliability, and compassionate approach will support healing, prevent complications, and improve patients’ overall quality of life.

    Key Responsibilities

    • Perform focused wound assessments, including size, appearance, drainage, and signs of infection. • Identify and communicate changes in wound condition to supervising providers. • Carry out wound care plans as directed by supervising physicians or advanced practice providers. • Administer dressing changes, cleansing, packing, and other routine wound treatments. • Deliver Ultramist therapy following established clinical guidelines and safety protocols. • Assist with advanced wound care procedures as needed. • Reinforce treatment instructions to promote healing and support patient understanding. • Work closely with physicians, NPs/PAs, RNs, and facility staff to coordinate care plans. • Maintain accurate, timely, and detailed documentation of wound assessments, treatments, and progress in the EMR. • Upload wound photos, measurements, and treatment notes according to organizational standards. • Provide wound care in homes, SNFs, assisted living facilities, and clinic environments. • Represent Innovative Wound Solutions with professionalism, compassion, and respect in all interactions.

    Qualifications

    • Graduate of an accredited LVN program. • Active California LVN license in good standing. • Current CPR certification (AHA). • Minimum of one year of clinical experience as an LVN, preferably with exposure to wound care in an outpatient or post-acute facility. • Prior wound care experience preferred; willingness to complete Ultramist training required. • Excellent communication and interpersonal skills to effectively interact with patients, families, and healthcare teams. • Ability to work collaboratively as part of a multidisciplinary team. • Strong commitment to compassionate, patient-centered care. • Reliable critical-thinking and problem-solving skills to address evolving wound care needs. • Dependable transportation and ability to travel throughout Riverside and San Diego counties.
