This full-time, in-office role is ideal for someone who enjoys wearing many hats, solving problems on the fly, and playing a key role in keeping our agency running smoothly.

This isn’t a typical desk job — you’ll help with everything from chart reviews and care coordination to compliance and patient follow-ups. If you're proactive, organized, and excited to be part of a team where your input truly matters, we want to hear from you.

💼 What You’ll Be Doing:

📝 Care Coordination & Support – Assist with patient records, care plan updates, and survey prep

☎️ Patient & Provider Communication – Follow up on orders, coordinate with doctors, and help patients get the services they need

🗂️ Compliance & Quality Assurance – Help ensure documentation is accurate and that we’re always ready for audits or visits

🔄 Jump In Where Needed – Support intake, handle clinical questions, assist the administrator or DON — be the reliable go-to

👀 Who We’re Looking For:

💼 Licensed LVN or RN in Texas

📆 1–3+ years of nursing experience (home health experience preferred but not required)

💡 Independent & Adaptable – You’re comfortable working without constant supervision and love variety

🤝 Excellent Communicator – You work well with patients, doctors, and teammates

🧠 Detail-Oriented – You’re great at spotting what’s missing and making sure nothing falls through the cracks

🌎 Spanish-speaking is a plus, but not required
