Position Highlights:

One-on-one patient care

Responsibilities:

Providing skilled nursing care in the home setting

Medication administration and treatments

Monitoring and documenting patient condition

Collaborating with physicians, families, and care teams

Following individualized care plans

Ensuring patient comfort, safety, and well-being

Requirements:

Current Texas LVN license in good standing

Strong clinical and communication skills

Compassionate, dependable, and professional attitude

Home health or private duty experience preferred

Current CPR certification preferred

Reliable transportation
