From our genesis as a community health center in 1971, we understood that we cannot heal our communities without also addressing the economic and social factors that affect health. Many times, this requires that we go outside and provide services in other settings to build the bridge that links individuals to our health center for preventive and primary care. Over 90,000 individuals come to La Clínica because it is a welcoming place that addresses the whole person, coordinating and connecting them to a broad network of services to improve and maintain their health and well-being. While we are still known for our activism and spirit of social justice, we are also proud to have grown into a sophisticated provider of primary health care services with 35 sites across Alameda, Contra Costa and Solano counties.  BE PART OF AN EXCELLENT CARE TEAM: We are looking for Panel Management Assistants who are inspired to deliver a great experience to our patients and who want to play an important role in patient-centered care by performing at the top of their scope of practice. This position will work to improve health outcomes of patients through population health and panel management. Through the use of robust patient registries, this position will identify unmet care needs, and in collaboration with the health care team, target interventions to improve the health outcomes of the patients. The PMA will assist patients in accessing targeted care, improve the quality of chronic care service delivery, assist medical providers in care management, manage La Clinica's population health management system (i2i), and participate in continuous quality improvement. This position will perform all general duties of a Medical Assistant as required. other care team members.   

    MAJOR AREAS OF RESPONSIBILITY INCLUDE BUT ARE NOT LIMITED TO:  PATIENT CARE

    • Update and maintain registry of patients, using robust tracking system, that can follow patients based on panels belonging to individual primary providers OR disease /condition.
    • Enter data and manage the integrity of data entries for patient registries by conducting data cleanup, running periodic reports, and confirming accuracy of electronic registry records.

    • Systematically review registries for target populations/panels, to conduct predetermined interventions agreed upon by health care team.

    COMMITMENT TO QUALITY 

    • Carry out provider/health care team orders (those consistent with level of training). Activities may range from messenger functions such as ordering labs, scheduling appointments, coordinating referrals, or may include more complex health coaching responsibilities.. May use standing orders to address common problems.
    • Participate in health coaching including activities such as: medication adherence, counseling patients on healthy behaviors, providing patients with support on self-management goals, and referrals to access a range of healthcare services.
    • Contribute to quality improvement efforts by providing feedback and progress reports to Site Improvement Teams on health status and outcomes of panels. PMA may also serve as a member of the Site Improvement Team.
    • Collaborate with other health care team members, such as health educators or BMS, to support individual or group based interventions of care.
    • Attend section meetings and other meetings that support PMA activities.
    • Carry out the full array of Medical Assistant duties as assigned based on operational needs of the clinic.
    • Performs other duties as assigned.

    GENERAL OFFICE DUTIES

    • Perform general office duties, such as scheduling appointments for patients, answering telephones, recording medical updates from the provider, or completing insurance forms.
    • Screen and monitor EHR requests, completing or routing as assigned.
    • Greet and log in patients arriving at office or clinic in the appropriate electronic system.
    • Contact medical facilities or departments to schedule patients for tests or appointments.
    • Inventory stock, order, and verify receipt of medical, lab, or office supplies or equipment.
    • Keeps equipment operating by following operating instructions; troubleshooting breakdowns; maintaining supplies; performing preventive maintenance; calling for repairs.
    • Enter key medical record information or perform other required record keeping duties required.

    KNOWLEDGE 

    • Demonstrated knowledge and experience managing patient databases and other computer software programs such as Excel, Outlook, and practice management systems.
    • Demonstrated knowledge of chronic care model, fundamentals of health education, motivational interviewing and supporting patients with self- assessment goals desired.

    ABILITIES

    • Self-driven individual with ability to work independently and exercise good judgment. Also able to work effectively with teams, demonstrating willingness to collaborate to improve services, care management and data processes.
    • Ability to protect and maintain strict confidentiality regarding patient information.
    • Demonstrated ability to work and communicate with people from various ethnic, socio-economic, educational and experiential backgrounds.
    • Availability during regular clinic hours including evenings and Saturdays may be necessary.
    • Bilingual English/Spanish required.

    OTHER CERTIFICATIONS AND EXPERIENCE

    • Requires high school diploma or equivalent and completion of an accredited Medical Assistant (MA) program plus one year hospital/clinic/medical office experience.
    • State or National Certification as a Medical Assistant
    • Experience working with patients in the capacity of health promotion, with knowledge of medical terminology, chronic health conditions, and health coaching.
