Lead Registered Nurse (RN) Care Manager needed in Allentown, PA.

Salary and Benefits
- The salary range is based on experience ($74,000 - $100,000/year)

Responsibilities
- Monday – Friday (8:00 am – 5:00 pm)
- Ability to work one day a week remotely after training
- Provide clinical care management support across OBGYN, Pediatrics, and Internal Medicine/Family Medicine
- Conducts assessments of patient/family needs for the highest risk patients at risk for poor outcomes
- Collaborate with the care team in assessing, planning, and implementing the patient’s care plan
- Provide daily clinical and operational leadership for the Care Management team by serving as the primary clinical resource, training and onboarding staff, and coordinating cross-department collaboration
Requirements
- Clear and active Registered Nurse license in the state of Pennsylvania
- 5 years’ experience of chronic management experience working in a care management capacity
- 1 year of experience or leadership experience preferred
- Bachelor of Science in Nursing (BSN) required
