    Position Summary:

    We are seeking a Full-Time Program Specialist responsible for overseeing a consumer caseload of as many as 10 individuals and providing program management for chapter 6100/6400. The Program Specialist will be responsible for management and oversight of the safety and well-being of individuals with intellectual disabilities, autism, mental health, and behavioral health needs in a residential setting and coordinating activities and community integration in accordance with everyone’s Individual Plan (IP).

    Primary Role & Responsibilities:

    Consistent with Community Homes for Individuals with Intellectual Disability Regulations, Chapter 6100/6400, the role of a Program Specialist will document the following as it pertains to the ISP process:

    Formulates individual goals for consumer on caseload, to be discussed at the Annual ISP meeting and reviewed quarterly.

    Completes annual assessment for each consumer on caseload in accordance with regulatory requirements.

    Provides the assessment for the development of the ISP, ISP Annual Update, and all ISP revisions.

    Fulfils the role of the Plan Lead.

    Reviews the ISP, ISP Annual Update, and all ISP revisions for content accuracy.

    Reports content discrepancies to the support coordinator and plan team members.

    Ensures that the ISP is implemented as written.

    Supervises, monitors, and evaluates those services are being provided as written in the ISP.

    Reviews, signs, and dates the monthly documentation of an individual’s participation and progress towards outcomes.

    Reports changes related to the individual’s needs to the support coordinator, as applicable, and plan team members.

    Reviews the ISP with the individuals, as required, and document the review.

    Provides documentation for the plan review to the support coordinator, as applicable, and plan team members.

    Informs plan team members of the option to decline the ISP review documentation.

    Recommends revisions to services or outcomes in the ISP to the support coordinator and plan team members.

    Coordinates the services provided to the individual pertaining to the residential program.

    Follow up on Monthly Monitor Concern sheets with Residential Supervision.

    Attend program meetings, Team meetings, ISP Meetings, SIS Meeting, Behavior support meetings, etc.

    Provide and supervise activities for the individuals in accordance with their specific IP, including opportunities for community integration.

    Hold regular meetings with Team Leads and Direct Support Professionals to review program needs/progress and provide necessary training.

    Assure budgetary compliance and fiscal accountability for all residential programs.

    Ensure program adherence to established policies and procedures.

    Provide the assessment for the development of the IP, the IP Annual update, and the IP revisions as required under CH. 6100/6400.181(a).

    Perform other duties and responsibilities as assigned.

    Required Skills and Knowledge of The Program Specialist:

    Minimum of two years supervisory experience

    Valid driver’s license

    Experience with 6100/6400 regulations

    Excellent written and verbal communication skills

    Working knowledge of Therap

    Master’s degree or higher from an accredited college or university and one (1) year experience working directly with individuals with developmental disabilities.
