Job Summary
Responsible for providing case management services to youth or adults with serious emotional disturbances as defined by the State of Montana. Case management services will be provided to support clients in gaining access to needed services within the community as needed and convenient for the client. All duties will be conducted under the guidance of the Program Supervisor.
Responsibilities and Duties
Advocate for the client inside and outside of L’esprit Inc. regarding needed services
Maintain constant communication to ensure that all services are being monitored and advocate for the client if this is not happening
Be able to engage with client and client’s family and develop a professional therapeutic relationship with boundaries
Be able to maintain calm and confident during a client’s crisis and maintain safety for both client and staff
Be able to follow plan for emergency evaluation and management of crisis
Provide assessments and service plans for client and client’s family
Create engaging activities to enhance the client and client’s family in a therapeutic setting
Provide progress notes in a timely manner regarding all contact with the client or on behalf of the client. Any significant changes in the client’s condition will be noted in a timely manner
Be prepared for quarterly reviews for service assessment

Must adhere to L’esprit Inc. policy and procedure including productivity and case load expectations and ethical and confidentiality expectations while following the code of ethics
Provide monthly supervision, face to face at least once a month with family support specialist, therapeutic adult mentor and home support specialist
Follow PQI standards
Be aware of personal mental health needs and model a healthy lifestyle
Be responsible for additional responsibilities as needed or assigned by supervisor
Qualifications and Skills
Minimum qualifications for a Case Manager are an Bachelor’s degree in human services field with at least one year full-time experience serving people with mental illnesses. Individuals with other educational backgrounds who, as providers, consumers, or advocates of mental health services have developed the necessary skills, may also be employed as intensive case managers. Valid Montana driver’s license and good driving record are essential.
