    Central Coast Home Health is a fast-growing, locally owned and operated organization with a great reputation throughout the Central Coast. We are currently seeking a full time RN or LVN Liaison to join our team in San Luis Obispo and Northern San Luis Obispo County.

    The regular work schedule is M-F, 8 AM-5 PM

    The hospice liaison is responsible for forging and maintaining positive relationships with customers and referral sources. The liaison must be comfortable and confident in introducing and discussing hospice and palliative services to families and patients in a hospital setting. They will collaborate with referral sources to establish an individual plan for each patient to admit onto hospice services. The liaison will be able to work closely with the hospice intake team on new hospice referrals. They will also participate in the broader marketing team and assist with community events and educational opportunities.

    Requirements:

    • Current California RN or LVN license
    • Minimum one (1) year of professional nursing work experience
    • Two years of experience in hospice or home health operations
    • Ability to market aggressively and deal tactfully with customers and community
    • IV Certification
    • Reliable vehicle to drive for work
    • Current Driver's License and maintenance of a clean driving record
    • Have and maintain auto insurance coverage
    • Current approved health care provider CPR Certification (AHA)
    • Home Health experience preferred but not required
