Job Summary
We are seeking a Licensed Practical Nurse (LPN), Registered Nurse (RN) to join our team. In this position, you will assist patients with medication management, treatments of care as per care plan, and supervise Direct Support Staff working with you on shift. Required a nurse with experience on long term facilities.
Responsibilities
. Patient Care & Monitoring
Assess patients’ health conditions upon admission and throughout their stay.
Monitor vital signs (blood pressure, pulse, temperature, respiration, oxygen saturation).
Provide direct nursing care such as wound care, IV therapy, G-tube feeding, tracheostomy care, and catheter management.
Assist patients with mobility, transfers, and daily living activities as needed.
Manage post-surgical and rehabilitative care.
. Medication Management
Administer medications safely (oral, IV, injections, topical, etc.).
Monitor for adverse drug reactions or side effects.
Document medication administration in patient charts.
. Care Planning & Documentation
Develop individualized care plans in collaboration with physicians, therapists, and families.
Document patient progress, treatments, and interventions.
Update care plans based on patient recovery and rehabilitation progress.
. Collaboration with Healthcare Team
Work with physical, occupational, and speech therapists to coordinate rehabilitation care.
Communicate patient progress and concerns with physicians.
Supervise and support CNAs, HHAs, and other care staff.
. Patient & Family Education
Educate patients and families on discharge instructions, home care, and rehabilitation exercises.
Provide emotional support to patients adjusting to illness, injury, or disability.
. Compliance & Safety
Education & Licensure
RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) + active RN license.
LPN/LVN: Completion of an accredited practical nursing program + active LPN license.
Certifications (Preferred/Required by Some Facilities)
CPR and Basic Life Support (BLS) certification.
Advanced Cardiac Life Support (ACLS) (for some RN roles).
Wound Care Certification, IV Therapy Certification (a plus).
Geriatric Nursing Certification (preferred in long-term/rehab care)
