Job description:
Job Overview Applicant must be fully licensed LCSW or LCHMC and must be licensed to practice in North Carolina. We are unable to accept provisional licensed staff for this role. You must be comfortable working with the justice involved population. The person will work 40 hours a week in Greensboro and High Point detention center providing project oversight, clinical assessments, and brief interventions to the participants in the program. This person will also be responsible for overseeing and working collaboratively with a team of three (3) clinical staff consisting of LCAS, QP and Community Health Worker. The role will provide comprehensive case planning, record keeping, and providing necessary information to the researcher. The Clinical Case Manager will utilize evidence-based and best practice interventions including Cognitive Behavioral Therapy, Motivational Interviewing, and Trauma-informed care, tailored to the unique needs of our diverse population. Experience in working with SMI and SUD is also highly preferred. Will be responsible for oversight of all data and outcome report.
Responsibilities
Direct Care
Coordinate and oversee initial and ongoing assessments of clients at the Greensboro and High Point Detention Facility with mental health and co-occurring substance use disorders.
Work closely with the QP and LCAS to develop diversion and re-entry plans with recommendations to address the client's therapeutic needs through appropriate treatment.
Administrative
Will oversee services provided by all team members.
Collect and provide data as required and participate in program evaluation.
Work collaboratively with a multi-disciplinary team in the criminal justice system.
Foster and maintain positive relationships with community treatment providers to support continuity of client care.
Participate on committees and community meetings as needed to educate community partners about the initiative.
Requirements
Must be a licensed LCSW or LCMHC
Experience in working with SMI and SUD is also highly preferred.
Experience with the Criminal Justice system is preferred although not required.
Experience working with justice-involved or formally incarcerated persons preferred.
Ability to work flexibly in a fast-paced work environment with multiple demands.
Ability to navigate complex systems and engage individuals in services.
Working knowledge of available community resources.
Ability to work collaboratively with multiple stakeholders.
Excellent written and oral communication skills.
Proficient in Microsoft and word processing applications
Strong attention to detail and ability to collect and report data.
Reliable means of transportation to complete job functions
