ABOUT THE ROLE: The Social Worker will be responsible for delivering trauma-informed, culturally responsive clinical services to justice-impacted individuals participating in Changing Perceptions’ programs. Reporting to the Program Director, the Social Worker will provide assessments, therapeutic interventions, and crisis support that promote wellness, emotional stability, and positive life outcomes. As a critical part of our care team, the Social Worker will collaborate with case managers and external providers to ensure wraparound support for each participant’s unique journey toward healing and reintegration.
WHAT YOU’LL DO:
As a Social Worker, you will:
Services & Therapy
Provide individual and group sessions to returning citizens using evidence-based and strengths-based approaches.
Offer crisis intervention and emotional stabilization support as needed
Maintain a caseload of clients and document all sessions in compliance with clinical standards.
Collaboration & Coordination
Collaborate with case managers, mentors, and external partners to ensure holistic and coordinated care.
Participate in multidisciplinary team meetings to track progress and refine service plans.
Provide referrals to psychiatric, substance use, and social service supports as appropriate.
Community & Participant Engagement
Support outreach and engagement strategies for individuals who may be hesitant to seek clinical services.
Facilitate therapeutic groups focused on mental wellness, reentry stress, trauma, and interpersonal skills.
Build rapport with participants through a trauma-aware, non-judgmental, and culturally grounded approach.
Training & Capacity Building
Educate staff and volunteers on mental health issues impacting justice-impacted populations.
Develop culturally appropriate mental health resources for participants and their families.
Serve as a thought partner on program design, integrating mental wellness across services.
Compliance & Documentation
Ensure all documentation is HIPAA-compliant and meets standards for clinical care delivery.
Stay current with best practices and licensing requirements.
WHAT WE’RE LOOKING FOR: Changing Perceptions may be the right fit for you if:
Have at least 3–5 years of experience, preferably working with justice-involved or trauma-impacted populations.
Demonstrate a deep understanding of the barriers faced by returning citizens and a strong commitment to equity and second chances.
Have experience facilitating therapeutic groups and managing caseloads.
Are skilled in crisis response, motivational interviewing, trauma-informed care, and cognitive behavioral therapy.
Excel at building trust across lines of difference and creating a safe, empowering therapeutic environment.
Are detail-oriented, tech-savvy, and capable of balancing administrative and clinical responsibilities.
Are comfortable working in community-based settings and flexible with participant needs.
