Position Summary
We are seeking a compassionate and mission-driven Licensed Clinical Social Worker (LCSW) to join our interdisciplinary care team at a growing Federally Qualified Health Center (FQHC) in Brooklyn, New York. The LCSW will provide behavioral health services to a diverse patient population, including children, adolescents, adults, and families, while supporting the organization's commitment to delivering high-quality, integrated, and culturally competent healthcare.
This role is ideal for a clinician who is passionate about serving underserved communities and collaborating with primary care providers to address the behavioral, emotional, and social determinants of health impacting patients.
Key Responsibilities
Clinical Services
Conduct comprehensive psychosocial assessments and behavioral health evaluations.
Develop, implement, and monitor individualized treatment plans.
Provide evidence-based individual, family, and group psychotherapy.
Diagnose and treat a wide range of mental health conditions, including anxiety, depression, trauma-related disorders, and adjustment disorders.
Conduct crisis intervention, risk assessments, and safety planning as needed.
Maintain accurate, timely, and compliant clinical documentation in the electronic health record (EHR).
Integrated Care Collaboration
Work closely with primary care providers, psychiatrists, nurses, care managers, and other healthcare professionals to coordinate patient care.
Participate in interdisciplinary team meetings and case conferences.
Support patients in addressing social determinants of health, including housing, food insecurity, transportation, and access to community resources.
Facilitate referrals to specialty behavioral health and community-based services when appropriate.
Compliance & Quality
Adhere to FQHC, state, federal, and regulatory guidelines, including HIPAA and Article 28 requirements.
Assist with quality improvement initiatives and outcome tracking.
Maintain licensure and continuing education requirements.
Qualifications
Required
Master's Degree in Social Work (MSW) from an accredited institution.
Current New York State Licensed Clinical Social Worker (LCSW) license in good standing.
Minimum of 2 years of clinical experience providing psychotherapy and behavioral health services.
Experience working with diverse and underserved populations.
Strong assessment, crisis intervention, and treatment planning skills.
Proficiency with Electronic Health Records (EHR).
Preferred
Experience in an FQHC, community health center, outpatient clinic, or integrated care setting.
Bilingual skills (Spanish, Haitian Creole, Russian, Mandarin, or other languages common in Brooklyn communities).
Knowledge of trauma-informed care, motivational interviewing, CBT, DBT, and other evidence-based treatment modalities.
