    • Deliver Comprehensive Support: Play a key role in our program designed to provide medical and social support to patients in their homes.
    • Create Care Plans: Develop care plans for chronically ill frail elderly seniors, focusing on their unique needs.
    • Educate and Advocate: Educate patients on the importance of health monitoring, medication compliance, and managing chronic conditions.
    • Collaborate on Care: Work in weekly interdisciplinary team meetings to develop tailored care plans and address potential care gaps.
