Licensed Physical Therapist (Home Health) – Lake Worth Area or Boynton Beach to Delray Area

About the Role: M.D. Home Health, LLC is seeking a Licensed Physical Therapist to provide in-home care throughout the Lake Worth Area or Boynton Beach to Delray Area. This is a flexible, patient-centered opportunity for a clinician who values autonomy, efficiency, and meaningful patient impact.

You won’t be stuck in a clinic juggling 3 patients at once—you’ll deliver one-on-one care where it actually matters: in the patient’s home.

BONUS OPPORTUNITY: Earn up to $1,500 in performance-based bonuses for maintaining a consistent caseload, with additional incentives for servicing high-demand areas.

What You’ll Do:

Perform in-home evaluations and reassessments to develop individualized plans of care

Deliver skilled therapy interventions to improve mobility, strength, balance, and independence

Provide fall prevention, gait training, and functional mobility training

Educate patients and caregivers on home exercise programs and safety strategies

Communicate with physicians and interdisciplinary team members to ensure coordinated care

Complete documentation in a timely manner using our EMR system

Manage your caseload efficiently within your assigned territory

What We’re Looking For:

Active Florida Physical Therapist License (Required)

Minimum 1 year of clinical experience (home health preferred, but not required)

Strong clinical decision-making and time management skills

Ability to work independently in the field

Reliable transportation and willingness to travel within assigned coverage area

Comfortable with EMR documentation

CPR certification
