Responsibilities: • Lead and coordinate daily nursing activities to ensure high-quality care for our residents. • Collaborate with the Registered Nurse to develop and implement personalized care plans. • Monitor and document residents' health status, ensuring timely interventions when necessary. • Provide compassionate support and guidance to residents and their families, fostering a warm community. • Supervise and mentor nursing staff, promoting a culture of continuous learning and improvement. • Ensure compliance with state and federal regulations, maintaining the highest standards of care. • Act as a liaison between residents, families, and healthcare providers to facilitate effective communication and care coordination. Qualifications: Requirements: • Must be licensed with the State of Wisconsin or Minnesota as an LPN, and must maintain current unencumbered licensure. • Previous experience in a related field. • Maintains current background check on file with the State of Minnesota or Wisconsin and agencies involved. • Must have a valid Minnesota or Wisconsin Driver’s License. • Drives company and personal vehicle in a safe manner and in accordance with MN and WI State Laws. • Must be able to read, write, and communicate in English. Compensation: $30 - $33 hourly
