Responsibilities

- Assist with patient assessments and communicate changes in condition to the appropriate clinical team members.
- Support physicians, registered nurses, and other care team members in delivering coordinated care.
- Maintain accurate and timely medical records and charting.
- Educate patients and family members on care instructions, medications, and wellness practices as directed.
- Follow infection control, safety, and regulatory standards to promote a safe care environment.
- Respond appropriately to patient needs, concerns, and emergencies within the scope of practice.
- Contribute to a respectful, patient-centered, and professional workplace culture.
Qualifications
- Current practical nursing licensure in the applicable state or jurisdiction.
- Experience providing patient care in a clinical, long-term care, home health, hospital, or similar setting preferred.
- Strong understanding of nursing principles, patient monitoring, and basic clinical procedures.
- Ability to document care accurately and use electronic health record systems effectively.
- Excellent communication and interpersonal skills.
- Ability to prioritize tasks, remain organized, and respond calmly in changing situations.
- Commitment to delivering respectful, compassionate, and patient-focused care.
- Current CPR certification or willingness to obtain may be required.
