    Responsibilities

    • Provide direct patient care in accordance with established care plans, policies, and procedures.
    • Monitor and document patient conditions, vital signs, symptoms, and responses to treatment.
    • Administer medications and treatments as assigned and permitted by licensure and facility guidelines.
    • Assist with patient assessments and communicate changes in condition to the appropriate clinical team members.
    • Support physicians, registered nurses, and other care team members in delivering coordinated care.
    • Maintain accurate and timely medical records and charting.
    • Educate patients and family members on care instructions, medications, and wellness practices as directed.
    • Follow infection control, safety, and regulatory standards to promote a safe care environment.
    • Respond appropriately to patient needs, concerns, and emergencies within the scope of practice.
    • Contribute to a respectful, patient-centered, and professional workplace culture.

    Qualifications

    • Current practical nursing licensure in the applicable state or jurisdiction.
    • Experience providing patient care in a clinical, long-term care, home health, hospital, or similar setting preferred.
    • Strong understanding of nursing principles, patient monitoring, and basic clinical procedures.
    • Ability to document care accurately and use electronic health record systems effectively.
    • Excellent communication and interpersonal skills.
    • Ability to prioritize tasks, remain organized, and respond calmly in changing situations.
    • Commitment to delivering respectful, compassionate, and patient-focused care.
    • Current CPR certification or willingness to obtain may be required.
