Job Summary
Aveon Services is seeking a Licensed Practical Nurse (LPN) to conduct in-home assessments, supervise care staff, and provide ongoing training and support to ensure high-quality care for our clients. This role is ideal for nurses who are passionate about leadership, education, and making a meaningful impact in home healthcare. Part-Time or Full-Time
Key Responsibilities:
Conduct comprehensive in-home nursing assessments for new and existing clients
Develop and update care plans in coordination with RNs and the care team
Supervise, evaluate, and support caregivers (CNAs, CMTs, PCAs) assigned to clients
Provide onboarding and ongoing training to field staff on protocols, documentation, and client care
Monitor care delivery and ensure adherence to care plans and agency standards
Serve as a liaison between clients, families, caregivers, and agency leadership
Report changes in client condition or staff concerns to the RN or Administrator
📋 Duties Include:
Perform intake assessments, routine reassessments, and client check-ins in the home
Review care documentation submitted by staff; provide feedback and coaching as needed
Lead skills-based training sessions (e.g., ADLs, infection control, documentation)
Observe caregiver performance and provide field-based evaluations
Ensure compliance with infection control procedures and HIPAA regulations
Collaborate with the administrative team to continuously improve care quality
Respond to emergencies promptly and escalate per agency protocol
🎓 Qualifications:
Active LPN license with the Maryland Board of Nursing (Required)
Current CPR and First Aid certification (Required)
Physical exam and TB test completed within the past 12 months
Valid photo ID and Social Security card (front and back copies)
Minimum 1 year of LPN experience (home care or supervisory experience preferred)
Experience in caregiver training and staff development is a strong plus
Clean background check
Excellent communication, leadership, and documentation skills
Reliable transportation and the ability to travel throughout Maryland
