Job Summary Our home care agency is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care to clients in the comfort of their homes. The ideal candidate will be patient-focused, detail-oriented, and committed to delivering high-quality care that enhances the health, safety, and well-being of each client.

Key Responsibilities

Provide direct nursing care, including medication administration and treatment as prescribed.

Monitor and document clients’ health status, progress, and any changes in condition.

Collaborate with physicians, family members, and other healthcare professionals to ensure continuity of care.

Educate clients and families on care plans, health maintenance, and home safety practices.

Respond promptly to medical emergencies and provide appropriate interventions.

Maintain accurate clinical notes and submit required documentation on time.

Ensure compliance with state, federal, and agency policies, including infection control and safety procedures.

Qualifications & Requirements

Valid LPN license in the state of [insert state].

Current CPR and First Aid certification.
