Key Responsibilities:

Provide skilled nursing care in accordance with care plans and physician orders

Assist with medication administration and wound care

Monitor patient conditions and report any changes to the RN/Clinical Director

Collaborate with interdisciplinary team members to ensure quality outcomes

Educate patients and caregivers on treatments and wellness practices

Document care and observations accurately in the EMR system

Communicate with physicians, case managers, and other team members as needed

Qualifications:

Current LPN license in the state of Indiana (required)

Minimum 1 year of home health, hospice, or clinical nursing experience preferred

Strong assessment, documentation, and time-management skills

Reliable transportation and valid driver’s license (for field visits)

Passion for helping others and making a difference in patients’ lives

EMR experience a plus (Axxess preferred but not required)
