    Job Summary We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide hands-on nursing support to individuals with Intellectual and Developmental Disabilities (IDD). Under the supervision of a Registered Nurse (RN) or physician, the LPN will deliver person-centered nursing care that promotes health, safety, dignity, and independence in community-based or residential settings. Key Responsibilities • Provide direct nursing care in accordance with physician orders, RN supervision, and individualized service plans. • Monitor vital signs and observe physical or behavioral changes. • Administer medications and treatments within LPN scope of practice. • Assist with tube feedings, catheter care, ostomy care, and infection control procedures as trained. • Report changes in condition promptly to supervising RN or provider. • Support health education for individuals and direct support staff. • Respond appropriately to medical incidents or emergencies. • Maintain accurate and confidential medical documentation in compliance with HIPAA. Required Skills & Competencies • Experience or strong interest in working with individuals with IDD. • Knowledge of medication administration and basic nursing procedures. • Familiarity with EHR documentation systems. • CPR/BLS knowledge and emergency response skills. • Strong communication, teamwork, and compassion.
